Arrival and Check-In

  • General Instructions
    When you arrive for your appointment, please inform the reception desk that you are here to see the Exercise Physiologist. They will provide you with access to the center and/or gym.

  • Ashburton Location
    After entering the gym, please wait on the seats outside Consultation Room 2 until it is time for your appointment.

  • Carlton Location
    Once you have checked in at reception, please wait in the welcome area by reception until your practitioner is ready to see you.

     

Cancellation Policy
Lifestyle & Health Co is committed to providing exceptional care. When you book an appointment, that time is reserved exclusively for you and is no longer available to other clients who may need care.

  • Notice Requirement
    If cancellation is necessary, please notify us by email at admin@lifestylehealthco.com or call us at 1800 943 442 at least 24 hours in advance.

  • Late Cancellations
    A cancellation is considered late if made less than 24 hours before the scheduled appointment time. In such cases, the respective consultation fee will be charged.

  • No-Show Policy
    A no-show occurs when a patient fails to attend an appointment without canceling. In this instance, the respective consultation fee will also be charged.

Providing 24 hours’ notice allows us to offer your time slot to another client who may be waiting for an appointment.
Billing Policy

  • Invoicing Schedule
    Invoices for services rendered are issued once a fortnight.

  • Payment Terms
    Payment is due within 7 days of the invoice date.

  • Late Fee
    If payment remains outstanding 40 days after the invoice due date, a 10% late fee will be applied to the outstanding balance.

     

Settlement of Accounts
In the event that a third party (e.g., Medicare, DVA, NDIS, or an insurer) declines to pay the consultation fee for any reason, the client is responsible for the payment of any outstanding fees. Lifestyle & Health Co does not assume liability for costs rejected by third-party payers.

Medication
If you are medically required to carry medication for your condition and safe exercise inclusion (e.g., asthma relievers, nitroglycerin spray), please ensure you bring these medications with you to each appointment. Having quick access to essential medications is critical for your health and well-being.

Home Visits
We generally encourage clients to attend appointments in our clinic, where we have full access to the necessary equipment. However:

  • Eligibility & Booking
    Home visits may be available to regular clinic patients living within the local area, subject to practitioner discretion. These appointments must be paid for in advance.

  • Safety Considerations
    If your practitioner deems the home environment to be unsafe, they reserve the right to cancel or end the appointment early. In this instance, you will still incur the full session fee.
